Tutopatch and fibrin glue application in scleral fixation IOL

Purpose:

to investigate tutopatch and fibrin glue application in scleral fixation intraocular lens (IOL) implant to prevent suture extrusion in replacement of conventional scleral flap.

Setting:

National University Hospital, Singapore

Methods:

Retrospective case observational study of twelve patients who underwent modified technique of scleral fixation IOL implant. In our technique, scleral flaps are not created and conjunctiva peritomies are closed without sutures. In all cases in our series, Tutopatch (Tutopatch, Tutogen) was used to cover the suture knots after direct scleral placement and Fibrin glue (Tisseel, Baxter) was applied to the undersurface of Tutopatch and on the open sclera to provide adherence or closing the conjunctival peritomy. The patients are observed for Tutopatch visibility and suture/knot conjunctival extrusion and extrusion related problem during 1 year follow up.

Results:

Eleven out of 12 patients had the Tutopatch visible under the conjunctiva up to 3 months and became non-visible after 6 month post operatively. No conjunctival knot extrusion was seen at 1 year post op for this 11 patients observed. 1 out of 12 had tutopatch melt and scleral suture conjunctival extrusion in 1 month, which underwent repair. No extrusion related problem seen within 1 year

Conclusions:

Use of tutopatch and fibrin glue in scleral fixated IOL implant provided protection from suture extrusion in 1 year post op. It provides convenience, but more expensive method rather than scleral flap conventional method. FINANCIAL DISCLOSURE?: No
