Purpose:
To determine in patients taking tamsulosin and who underwent cataract surgery, the effect of intracameral phenylephrine prophylaxis on the incidence and degree of IFIS.
Setting:
Department of Ophthalmology, Hospital Parc Taulķ Sabadell, Universitat Autņnoma de Barcelona, Catalunya
Methods:
We reviewed the records of all patients cataract surgery between January 2009 and July 2010. Notes were studied of the operation for patients taking tamsulosin at the time of surgery. IFIS was defined and classified by the presence of any of the three phenomena that constitute the syndrome: the presence of undulation of the iris prolapse of the iris and pupil constriction. Surgical times were recorded, incidents and complications.
Results:
Review of 3156 records identified 64 patients (99 eyes), taking tamsulosin at the time of surgery. The overall incidence of IFIS in patients undergoing total was 3.1%. Those eyes received intracameral phenylephrine prophylaxis, the incidence of IFIS was 12%. Those eyes received intracameral phenylephrine prophylaxis, the incidence of IFIS was 12%. The incidence of IFIS was significantly associated with IFIS (p = 0.01). Iris hooks were used in 6 eyes (6.1%). The rupture of the posterior capsule with vitreous loss occurred in two eyes (2%) in which intracameral phenylephrine was not used.
Conclusions:
The incidence of IFIS in the group without use of phenylephrine was similar to other publications. Prophylactic intracameral phenylephrine reduced the incidence and degree of IFIS. In this group the operating time was shorter and there were fewer complications. In our experience the use of phenylephrine did not increase the pupil diameter. FINANCIAL DISCLOSURE?: No
