Purpose:
To show the possibility and safety of using Nd: YAG-laser for complicated cataract extraction with wave length 1.44 mcm at patients eyes with silicone in vitreous cavity.

Methods:
At eyes of 23 patients at age from 25 to 67 years with silicone in vitreous cavity was used as bimanual methods of laser cataract extraction on device Nd: Yag 1.44 mcm which is laser of Russian production. Such methods provide capsulorhexis, laser lens destruction with fragments aspiration and cortical mass through 2,2 and 0,8 mm micropricks for aspirating/irrigative and laser handipieces. In 3 cases were used iridoretractors, in 4 cases were implanted tension rings. In all cases IOL were implanted. The operation was carrying out under cover of viscoprotector. The silicone in the form of bubbles, which got into anterior chamber, was aspirated.
Results:
In spite of absence of deviations of standard of carrying out the operation, in 10 cases during nucleus of lens fragments extraction or following its extraction in anterior chamber appeared bubbles of silicone, that in 7 cases connected with weakness or defects in ligamentous apparatus of lens at intact posterior capsular, but in 3 cases with defects presence of posterior capsular lens.
Intraoperative and postoperative complications were not mentioned. Loss of endothelial cells composed less than 6%. In spite of full silicone extraction from anterior chamber after in 4 cases appeared again silicone bubbles in anterior chamber, that didnt influence on retinal position and corneal condition in observable postoperative period. Visual acuity in every case was maximal and depended on individual initial condition of retina.
Conclusions:
Using Nd: Yag laser 1,44mcm for complicated cataract extraction at patients eyes with silicone in vitreous cavity is possible, safe and provides maximal postoperative functional result, which depends on initial changes on eye fundus. FINANCIAL DISCLOSURE?: No
