Intracameral pupil dilating technique

Purpose:

To evaluate the sequential use,safety and the appropriate technique of using intracameral lignocain and epinephrine to dilate the pupil for cataract and intraocular lens (IOL) implant surgery. A well dilated pupil is an important pre requiste of cataract surgery safely to proceed till it ends.

Setting:

Ophthalmology Department Saudi German Hospital Riyadh

Methods:

A total of 45 patients male and female were included in this study, which is hospital based observational and interventional study. All the patient underwent a complete systemic and ophthalmic evaluation and consent before planning the procedure. Lignocain 1% was instilled into the anterior chamber and after a waiting period of about 30 -45 seconds a dilute solution of adrenaline was irrigated into the anterior chamber. In a short duration of 30-60 seconds the pupil dilated to its full potential.

Results:

This technique of dilating pupil during surgery is very useful, quick and reliable if followed carefully. This facilitated the continuous curvilinear capsulorrhexis, good phacoemulsification, safe removal of the cortex with clean capsular bag and implant of the posterior chamber IOL in the bag as the pupil remained dilated throughout the procedure in 85 % of the cases. In 15% of the cases there was a need to repeat this regimen.The pupil remained well dilated throughout the procedure. It can also be used for other intraocular surgical procedures which need good pupil dilation during surgery. Not only it provides good pupil dilation, it also gives good intraocular anesthesia and also prevents intraocular bleeding. It appears to be safe in patients having hypertension and diabetes mellitus as patients with these comorbid conditions are difficult to dilate.

Conclusions:

This technique of dilating pupil for cataract IOL implant surgery appears to be a valuable addition to the already existing techniques of pupil dilation for intraocular surgical procedures mainly cataract surgery and other vitreoretinal procedures which need a good pupil dilation during the surgical procedure.
