Purpose:

To study possibilities of a Russian laser unit «Rakot» in the hard cataract crushing by means of Nd-YAG laser 1.44 ?m wavelength.

Setting:

The S. Fyodorov Eye Microsurgery Complex Moscow, Sankt-Pitersburg¹, Russia .

Methods:

1700 operations was carried out in 1245 patients aged 65-93 years using the Russian laser unit «Rakot» (1.44 ?m Nd-YAG ). . Brown hard nuclei were observed in 142 eyes (8.3%). Cataract nigra was in 21 eyes (1.2%).

Results:

Brown cataracts could be crushed within 3 - 4 minutes. Black cataracts were hardest and crushed during 5 min. No difficulties took place in any cases as well as a necessity to pass on to a manual technique. Intraoperatively 14 ruptures of posterior capsule (0.8%) were noted. Postoperatively 7 cases of transitory hypertension (0.4%) were revealed. The maximum possible visual acuity was already observed 1-3 days after surgery.

Conclusions:

The Russian technology, developed in 1995, allows to crush hardest nuclei within 4-5 minutes. It is the only one type of energetic cataract surgery cracked the nucleus without manual fragmentation. This surgery can be performed in elderly patients because intraoperatively it is not necessary to press on nucleus. FINANCIAL DISCLOSURE?: No
