Comparison of the correlation between Humphreys blue on yellow and white on white visual fields with Heidelberg retinal tomography III (HRT III)

Purpose:

a) Correlation of blue on yellow visual fields with HRT III in glaucoma patients and/or suspects. b) Correlation of white on white visual fields with HRT III in glaucoma patients and/or suspects. c) To find which of a) or b) correlates best with HRT

Setting:

Observational case series Patients attending glaucoma clinic of a tertiary eye care centre in southern India from May 2009 to March 2011, having glaucomatous changes in the optic nerve were included.

Methods:

Patients with reliable visual fields (MD<10) from the glaucoma clinic of tertiary eye care centre from May 2009-March 2011 underwent the tests. All patients underwent standard automated perimetry- white on white (W/W) and blue on yellow (B/Y) done either using the 24-2 or the 30-2 test strategy on the Humphrey Field Analyser (Software version 4.0) and confocal laser scanning tomography using the Heidelberg Retinal Tomograph (Software version 3.1). All these tests were performed within a two week period and tests that had a low reliability were repeated. The parameters that were studied to establish correlation were mean deviation (MD), pattern standard deviation (PSD) of White on white (W/W) and Blue on yellow(B/Y) perimeter and rim area (RA), rim volume (RV), cup area(CA), disc area (DA), RNFL thickness of HRT-3.

Results:

150 eyes of 78 patients were studied with ages ranged between 25 to 82 years, 52 of 78 patients were males. Amongst the eyes studied 51% (n=76) were right eyes and 49% (n=74) were left eyes. Pearsons correlation (Table 1) and linear regression (Table-2) was studied for parameters of White on white (W/W) and Blue on yellow (B/Y) perimeter with those of HRT-3. Correlation coefficient was 0.304 between MD of W/W with RA and 0.264 with RV, and MD of B/Y with RNFL was 0.154. Linear regression analysis showed a coefficient value of 0.07 between MD of W/W with RA. Rest parameters showed little or no correlation.

Conclusions:

HRT-3 shows better correlation with white on white Humphrey visual fields as compared to Blue on yellow perimetry in patients with early glaucoma, hence HRT-3 can be used as an alternative for detection in patients that are unable to perform fields, and the parameters that can be used are RNFL thickness, Rim area and Rim volume. However the correlation between the perimetry and HRT is weak hence it cannot replace the perimetry. FINANCIAL DISCLOSURE?: No
