Removal of ophthalmic viscosurgical device from intraocular lenses

Purpose:

Bacterial contamination in the lens capsule during cataract surgery could induce postoperative endophthalmitis. To reduce the risk of endophthalmitis, it is important to remove potentially contaminated ophthalmic viscosurgical device (OVD) from under the intraocular lens (IOL). The efficiency of removal could relate to the adhesive properties between the OVD and the IOL. This study was conducted to examine the adhesive properties between two kinds of OVD and three different IOLs, and to investigate the efficiency of irrigation and aspiration (I/A) technique to remove OVD from under the IOL.

Department of Ophthalmology, Ehime University School of Medicine

Methods:

Japanese porcine eyes were used to evaluate I/A technique. Lensectomy was performed by phacoemulsification and I/A. Following implantation of each IOL (A, Eternity-X-60, Santen, Japan; B, Acryfold-VA60BBR, HOYA, Japan; C, Acrysof-SA60AT, Alcon, U.S.A.) in the lens capsule, OVD (A, Opegan-hi, Santen, Japan; B, DisCoVisc, Alcon, U.S.A) stained with fluorescein was inserted under the IOL. The cannula was alternately put on the horizontal edge of each IOL to tilt it (tapping method) four times for 5 seconds of I/A per tap. After I/A, residual OVD under each IOL was recorded using ophthalmic surgical microscopy.

Results:

In porcine eyes, less OVD A was retained under IOL A or B compared to IOL C. More OVD B was retained under all IOLs tested compared to OVD A.

Conclusions:

Adhesive properties between OVDs and IOLs were different among the IOLs. OVD on or under Eternity-X-60 or Acryfold-VA60BBR IOLs was easily removed compared to Acrysof-SA60AT. It is more difficult to remove DisCoVisc under an IOL. FINANCIAL DISCLOSURE?: No
