Purpose:
To present a case of erroneous corneal measurements, which led to inaccurate predicted intraocular lens (IOL) power.
Setting:
Department of Ophthalmology of the Vila Nova de Gaia/Espinho Hospital, Portugal
Methods:
A 41 year-old-woman, high myope (OD: -19.0D, OS: -17.0), underwent preoperative assessment for clear lens extraction. The predicted IOL power, using the IOL Master SRK-T formula, was -7.0 D in the right eye and -1.0 D in the left eye. Bilateral, simultaneous, surgeries were performed and a residual hyperopic refractive error of +1.00+2.00x110 OD and +2.5+1.75x90 OS was found.
Results:
Repeated corneal topography led to different keratometric measurements, and subsequently different IOL powers. After confrontation, the patient revealed that she had worn contact lenses during the preoperative assessment. The IOLs were replaced, and the patient has a binocular vision of 20/25 without spectacles.
Conclusions:
This case demonstrates that contact lens (CL) wear can induce important errors in the IOL calculation. Surgeons and patients must be aware of this problem, in order to avoid poor patient compliance of CL discontinuation in the preoperative assessment. FINANCIAL DISCLOSURE?: No
