Purpose:
To present penetrating keratoplasty surgery performed with femtosecond laser iFS 150, to evaluate the postoperative healing, complications, changes in the refractive error, UCVA, BCVA, pachymetry and maximal keratometry of patient, who underwent bilateral penetrating keratoplasty surgery because of keratoconus grade III-IV on both eyes. The right eye was operated on with use of the femtosecond laser and the left eye was operated in the standard way. To present effects and differences based on these different surgical techniques.

Methods:
58 year old patient with condition of keratoconus gr. 3-4 on both eyes underwent bilateral penetrating keratoplasty. The right eye was operated with use of the femtosecond laser and the left eye was operated in the standard way. Prospective 12 months follow up of the eye that underwent keratoplasty surgery performed with femtosecond laser in comparison of results with retrospective gained data of the postoperative process of the standard penetrating keratoplasty treated eye. Preoperative visual acuity and 1st day, 1, 6 and 12 months postoperative UCVA, BCVA, autorefractometry, pachymetry measurements, maximal keratometry readings and complications were obtained. Evaluation of differences between these two surgical methods has been completed.
Results:
Preoperative parameters right eye (RE) versus left eye (LE): UCVA: RE 2/60, LE 1/60, BCVA: RE, LE: correction does not improve vision, RE, LE: autorefractometry: impossible, pachymetry: RE 228, LE 333um, max. keratometry: RE 74,6D, LE 75.4D.
12 months follow up after surgery parameters changed as follows:
1st day after surgery: UCVA: RE 0,4, LE 2/60, 1 month after surgery: UCVA: RE 0,32, LE 0,1 , BCVA: RE -3,5/-5,0ax.60°=0,4, LE correction does not improve vision, pachymetry: RE 588um, LE 507um, max. keratometry: RE 52,5D, LE 50,6D.
12 months after the surgery: UCVA: RE 0,16, LE 0,1p., BCVA: RE -4,0/-1,0ax.160°=0,7, LE -5,0/-3,50ax.170°=0,6, autorefractometry: RE -3,75/-2,75ax.158°, LE -6,75/-5,25ax.5°, pachymetry: RE 558 um, LE 505um,max. keratometry: RE 49,8D, LE 47,0D.

Conclusions:
Penetrating keratoplasty with use of femtosecond laser iFS 150 enables to create different patterns in recipient and donor cornea on the principle " key and lock" .
The choice of the particular pattern is based on the actual state of patient's cornea. The treatment is in comparison with the standard keratoplasty more precise and gentle to the patients cornea. This enables better postoperative UCVA, BCVA, less irritation and reduces risk of rejection. Until now 4 more patients underwent this treatment in our Refractive Centre with very good results of UCVA and BCVA and without any complications. Although we have just begun with this treatment at this stage we can tell that when the treatment is performed by an experienced surgeon in combination with cooperating patient, it seems to be safe and full-valued alternative to standard treatment options. Although the effect is quite individual it helps patients to get good vision acuity with less irritation early after the surgery. FINANCIAL DISCLOSURE?: No
