Results of application of combined phacoemulsification technique in mature and hypermature cataract cases

Abstract Details

Purpose:

To show the effectiveness of Horizontal Phaco Chop and Woodcuter’s techniques combination during ultrasound phacoemulsification of hard cataracts.

Setting:

The research was provided during two years among 128 patients (140 eyes) of Ophthalmological Clinic of " Kanaker-Zeytun" Medical Center in Yerevan, Armenia. 71% of cases were diagnosed mature cataract and 21% of cases hypermature cataract.

Methods:

When applying the horizontal phaco chop technique the lens nucleus is fixed by phaco tip then the chopper is moved towards the phaco tip. When the woodcutter’s technique is used the lens nucleus is divided by phaco tip moving towards the fixed chopper with ultrasound on. During the combination of those two techniques at first the phaco tip is placed lower the center of the lens and is driven into the nucleus with ultrasound on and fixed and then the chopper is moved from the lens equator towards phaco tip for 2-3 milimeterms. The actions are reapplied in turn. When reached the center of the nucleus the actions are applied at the same time or only the chopper is used.

Results:

Combination of these two allows to divide the nucleus more easily since it gives ability to better control the nucleus mobility, to decrease the vacuum pressure, to use less ultrasound power. Complications were observed in 1.4% of cases. Average visual acuity after surgery was 20/25.

Conclusions:

The combination of these techniques during phacoemulsification of hard cataracts makes the surgeries safer and more controllable. FINANCIAL DISCLOSURE?: No
