Purpose:
The aim of this study is the assessment of surgical techniques, peroperative and postoperative complications and functional results in DMEK after penetrating keratoplasty in a prospective non-randomized study
Setting:
Eye Clinic of Medical Faculty, Masaryk University and University Hospital Brno Bohunice, Czech Republic
Methods:
The study included 15 eyes (15 patients) operated because of endothelial dysfunction after penetrating keratoplasty using DMEK method. We evaluated the operative technique and complications, postoperative complications, anatomical (OCT) and functional results (pachymetry, endothelial microscopy, topography and BSCVA) 1 month and 3 months after surgery.
Results:
Complete anatomical attach of the lamella 3 months after surgery was recorded in 14 of the 15 cases (93%). Pachymetry changed from 870 µm (SD198) to 570 mm (SD 120). The density of the endothelial cells from 3170 (SD380) on the donor cornea fell after 1st month to 2420 (by 26%) and after 3rd month to 1740 (by 46%).In the recovery period, there is no change of the original astigmatism. BSCVA 3rd postoperative month ranged between 0.9 and 0.1, depending on the functional status of the retina. All corneas except one were after the DMEK fully transparent.
Conclusions:
This new solution technology of endothelial dysfunction of transplanted corneas allows the rapid rehabilitation of the patient. Moreover it makes possible to implant without complications the intraocular lenses, including the toric ones, without fear of astigmatism changes in the case of the need of solving the opaque transplant FINANCIAL DISCLOSURE?: No
