Treatment of nuclear fragment disslocation during phacoemulsification by 20g sutureless vitrectomy

Purpose:

Estimation of results and complications of sutureless 20G vitrectomy in dealing with nucleus or parts of nucleus prolapse during phacoemulsification

Setting:

16 patients with nucleus or a part of nucleus prolapse during catarct surgery (phacoemulsification) has been operated in our Clinic in the period of 2010-2011 has been operated.

Methods:

Average age of this group is 72 (52-83). Preoperative visual acuity was ranged from hand motion to 2/60. Sutureless 20G pars plana vitrectomy was performed in all cases. Posterior chamber intraocular lens implantation was made during cataract surgery in 8 eyes. Vitrectomy was preformed immediately after cataract surgery in 6 eyes. Follow-up period was 9,5 (5-19) months.

Results:

In the group with vitrectomy performed immediately after catarct surgery visual acuity was better than 0,5 in 5 eyes (83%), and less than 0,1 in one eye (17%). In the group with vitrectomy performed after cataract surgery, in the second act, visual acuity was higher than 0,5 in 5 eyes (50%), 0,1-0,5 in 3 eyes (30%) and up to 0,1 in one eye (10%). Secondary glaucoma and uveitis were more often in the case vitrectomy made in the second act and this procedure solve them. Cystoid macular oedema has been noted in one case.

Conclusions:

Nuclear fragments disslocation in the vitreal cavity is serious complication of phacoemulsification. Sutureless 20G pars plana vitrectomy performed in the same act as a cataract surgery because of removing nuclear segments results in better postoperative visual acuity and less complications. Despite of associated pathology like corneal oedema and secondary glaucoma, sutureless vitreoretinal surgery show very well anatomical and functional result and shorter postoperative recovery.
