Meningeal carcinomatosis presenting with optic disc edema as first manifestation

Abstract Details

Purpose:

To present a rare clinical case in which meningeal carcinomatosis manifested with unilateral optic disc edema.

Setting:

Department of Opthalmology, Kavala General Hospital, Greece

Methods:

A 56 year old man underwent total gastrectomy for gastric cancer. During the ensuing 15 days he developed progressive reduction of right eye vision. On examination visual acuity was found to be 1/20 with refractive correction (c.c) on his right eye and 8/20 c.c on his left eye. Fundoscopy revealed severe optic disc edema in the right eye and normal left eye fundus. Computed tomography and magnetic resonance imaging scans of the brain and orbits were normal. A lumbar puncture was performed because the patient complained of intense headaches and had epileptic seizures. Cerebrospinal fluid testing showed atypical dysplastic cells consistent with meningeal carcinomatosis.

Results:

Cerebrospinal fluid analysis obtained from a patient with unilateral optic disc edema and seizures revealed atypical dysplastic cells diagnostic of meningeal carcinomatosis.

Conclusions:

The combination of progressive visual acuity loss and unilateral optic disc edema can be the first manifestation of meningeal carcinomatosis. FINANCIAL DISCLOSURE?: No
