Corneal endothelial cell analysis in keratoconus patients wearing different types of contact lenses. A prospective study

Purpose:

to record both quantitative and morphological changes occurring in the corneal endothelium of keratoconic patients wearing different types of contact lenses (soft, hybrid or RGP lenses) in 1 year follow-up period.

Setting:

Mansoura ophthalmic center Mansoura university

Methods:

The current study was carried out on 82 keratoconic corneas divided into four groups according to the type of fitted contact lens. They included 14,18,28 and 22 eyes fitted with soft contact lenses, hybrid lenses, daily wear low Dk RGP lenses and high Dk RGP lenses respectively. Endothelial photographs were taken of each eye using noncontact specular examinations at the initial phase and then images were taken in 3-month, 6-months, and 12-months follow-up visits. The images were printed out with the analyzed data, including endothelial cell density (ECD) and percentage of different cell morphology.

Results:

Analysis of the results reveled that patients wearing soft, hybrid or low Dk RGP lenses have significant changes in percentage of hexagonality and polymegathism at the end of follow up period in comparison to baseline values, while no significant changes between the baseline value of ECD and the value after one year. As regard the patients wearing high Dk, we demonstrated that the all values (ECD, hexagonality and polymegathism) did not change significantly during the follow up period.

Conclusions:

Our study indicates that the endothelial cell morphology in patients with keratoconus is significantly affected by contact lens wear in the form of decreased hexagonality and increased polymegathism especially with low-Dk contact lenses.
