Visual outcome of posterior chamber phakic IOL versus wavefront-guided LASIK in treating moderate to high myopia with or without myopic astigmatism

Purpose:

Evaluation of the visual outcome after posterior chamber phakic IOL versus wavefront-guided LASIK to correct moderate to high myopia with or without astigmatism.

Setting:

Horus Vision Correction Center.

Methods:

A prospective comparative non-randomized case series of 26 patients with bilateral moderate to high myopia with or without astigmatism were done. One eye of each patient was treated with wavefront-guided LASIK ( Visx S4 IR, AMO) and the other eye which was beyond LASIK correction was treated with posterior chamber phakic IOL ( ICL/Toric ICL, STAAR). Comparison of the preoperative and postoperative visual function including patient preference was done. Follow up was at 1 day, 1 week, 1, 3, and 6 months posoperatively.

Results:

Mean age was 24.7± 2.6 years. Males were 11 while females were 15 patients. Mean preoperative MRSE was -6.67±1.37 D and -11.87±2.12 D in LASIK and ICL groups respectively. Mean preoperative BCVA were 1.0 and 0.78 respectively. Both groups reached maximum vision after 1 month and stayed so for 5 months. At 6 months, mean UCVA was 0.93 and 0.84, mean BCVA were 0.98 and 0.9 in LASIK and ICL groups respectively. Efficacy was 0.93 and 1.1, safety was 0.98 and 1.2 in LASIK and ICL groups respectively. MRSE was >0.5 D in 22 LASIK eyes and in 17 ICL eyes. 2 LASIK eyes lost 1 line and 3 eyes gained 1 line while 12 eyes in ICL group gained one or more lines in BCVA. Contrast Sensitivity showed no significant difference in LASIK group and significant improvement in ICL group. 3 patients with large mesopic pupil (>8 mm) had no night symptoms in LASIK eye and night glare and haloes in ICL eye. 15 patients (58%) preferred ICL eye. There were no operative or postoperative complications in both groups other than postoperative spikes of elevated IOP in ICL group.

Conclusions:

Wavefront-guided LASIK and posterior chamber phakic IOL are effective, safe, and predictable in correcting moderate to high myopia. Posterior chamber phakic IOL can be considered as a safe alternative to LASIK in moderate to high myopia with or without astigmatism. FINANCIAL DISCLOSURE?: No
