Purpose:

the aa report the multiple surgical treatment of a glaucoma case complicated with anatomical malposition of ciliary bodies

Setting:

Ophthalmological department Versilia hospital Lido di Camaiore

Methods:

a young high hyperopic patient affected by drug resistant glaucoma in RE affered to our department. We performed in temporal order Iridotomy Cataract surgery,Cairns trabeculectomy,Posterior vitrectomy and ciliary bodies cyclophotocoagulation , Ahmed valve insertion. Preop UBM showed an inverted position of ciliary bodies, post trab UBM showed an inverted acqueous flow .

Results:

The IOP was constantly high even after last surgery; it decreased performing yag laser capsulotomy through the previous iridotomy.

Conclusions:

Glaucoma still represents a great challenge for the surgeon, the solution sometime being found in following a non orthodoxical pathway FINANCIAL DISCLOSURE?: No
