Management of ocular alkali burn injury with triple therapy (amniotic membrane transplantation, limbal transplantation, subconjuctival bevacizumab injection), a case report

Purpose:

An ocular alkali burn injury( Sodium-hydroxide) with opaque cornea and limbal ischemia which was treated by Amnoitic membrane transplantation(AMT), limbal transplantation(LT), and subconjunctival Bevacizumab(Avastin) injection is reported.

Setting:

Busan Paik Hospital, Busan, south Korea.

Methods:

a 50-year old female who had suffered from severe ocular alkali burns( OD: 60 degrees limbal ischemia, visual acuity was 0.2, OS: more than 270 degrees limbal ischemia, iris and lens was not visible, visual acuity was finger count.) was treated by bilateral AMT, immediately. and 2 weeks later, left eye was treated by LT. finally, because corneal neovascularization of left eye was extensive, Bevacizumab(2.5mg) was administered 2 times to left eye by subconjunctival injection.

Results:

Right eye :Visual acuity improved from 20/100 at face to 20/25. Left eye: visual acuity improved from finger count to 20/100. When LT used in combination with AMT, reepithalization was complete in whole cornea. but corneal neovascularization was extended to corneal center. total corneal neovascularization area was decreased to 30% in comparison with pre-Bevacizumab injection. finally corneal center relatively reserved clear.

Conclusions:

Ocular chemical injuries are a true ocular emergency and require immediate and intensive evaluation and treatment. this triple method will provide hope for patients. FINANCIAL DISCLOSURE?: No
