Comparison of difractive, refractive multifocal intraocular lens and monofocal monovision for cataract surgery

Purpose:

To compare visual outcomes and patient satisfaction in patients with bilateral aspheric diffractive multifocal intraocular lenses (IOLs), bilateral refractive IOLs, and patients with monofocal aspheric IOL monovision.

Setting:

Oftalmosalud Instituto de Ojos. Lima-Peru.

Methods:

This prospective comparative study comprised 90 consecutive bilateral cataract patients between April 2011 – October 2011. Bilateral Tecnis ZM900 multifocal IOL was implanted in 35 patients. Bilateral Rezoom multifocal IOL was implanted in 35 patients. Bilateral monofocal aspheric IOL was implanted in 20 patients (one eye target to plano and the other eye target to 1.5D). Parameters analyzed 3 months postoperatively included binocular uncorrected distance, and near visual acuities; spectacle independence; subjective visual symptoms; and patient satisfaction. Patients were administered the Visual Function Questionnaire-25 preoperatively and postoperatively.

Results:

The were not statistically significant difference between sex and age in the 3 groups. 79.9%, 71.4% and 75% in the Tecnis, Rezoom and monofocal monovision group achieved postoperative distance UCVA of 20/25 or better. 85.7%, 74.2% and 75% in the Tecnis, Rezoom and monofocal monovision group achieved postoperative near UCVA of J2 or better. There were not statistically significant difference bewteen the 3 groups (p > 0.05) in terms of near and distance postoperative UCVA. More than 85% of the patients in all 3 groups reported excellent overall spectacle independence and satisfaction after the surgery. 80 and 40% of the Monofocal monovision group reported not have halos and glare none of the time. Tecnis and Rezoom groups showed higher incidence on halos and glare compared with the monofocal monovision group.

Conclusions:

Multifocal IOLs and monofocal monovision showed good results in terms of distance and near vision, as well high level of spectacle independence. Multifocal group showed higher incidence on halos and glare compared with the monofocal monovision. FINANCIAL DISCLOSURE?: No
