Purpose:
To show that injectable Capsular Tension Ring (CTR) is the right method to successfully negotiate most difficult, traumatic, pediatric cataract with zonular deficiency.
Setting:
Private practice in suburban Mumbai, India
Methods:
9 years old child with severe trauma of at least 3 years duration was presented with traumatic cataract. There was calcified nucleus with crumpled anterior capsule. Posterior synchea with irregular pupil and sphincter damage. Approximately 180 degree zonular dialysis observed with only hand movement vision in the eye.
Pre-operative assessment with UBM, specular microscopy and Bscan were done to exactly decide the approach to surgical treatment. All types of IOLs viz. Scleral fixated, Glued, iris claw anterior chamber/posterior chamber were kept ready with anterior vitrectomy readiness. But Injectable CTR changed the game plan to a fascinating procedure.
Results:
Injectable CTR made the traumatized, fibrosed capsular bag regaining its contour and posterior chamber IOL stabilized the bag more firmly
Conclusions:
Thus CTR is most important surgical device to have a stabilised bag in Traumatic cataracts. FINANCIAL DISCLOSURE?: No
