Purpose:
To demonstrate the findings of a case of accommodative spasm and its corelation on pentacam
Setting:
Tertiary eye care centre
Methods:
Ophthalmic examination and investigations including manifest and cycloplegic refraction, pentacam examination for evaluating changes in anterior chamber depth (ACD) and lenticular thickness
Results:
A 16 year girl complained of sudden onset blurring of vision in both eyes following reading under dim light. She was a known myope with 2 diopter sphere OU. There was sudden increase of myopia to 4 dioptres sphere in one day. At presentation, on the third day since her symptoms, her best- corrected visual acuity (BCVA ) was 6/6 OU with manifest refraction of 7 diopter sphere and 7.5 / - 0.5 x 70 in the right and left eye, respectively. Clinical examination revealed miosis and shallow anterior chamber. Pentacam imaging depicted shallow anterior chamber with increased lens thickness. Cycloplegic refraction was 1.5 sphere in the right and -1.75 sphere in the left eye, and she was corrected to 6/6 OU. Scheimpflug imaging objectively demonstrated the increase in anterior chamber depth, decrease in lens thickness and and movement of the anterior as well as posterior lenticular surface.
Conclusions:
Spasm of accommodation should be suspected in a patient with acute increase of myopia. Pentacam imaging is a non invasive technique to depict the morphological changes in such a case FINANCIAL DISCLOSURE?: No
