Toric IOL implantation, is it a re-adjustable procedure? (Case presentation)

Purpose:

Toric IOLs are now considered one of the effective refractive procedures. Their technology has contributed to significant improvement of the visual outcome. Proper alignment of the axis within the bag is the most crucial step of the surgery which controls the precision of the results. Here I report a case of Toric IOL readjustment to re-align the axis 6 days post implantation.

Setting:

New Dar Al-Shifa Hospital

Methods:

A 42 years old female patient came for refractive surgery. Her pre-operative MR (Manifest Refraction) was Sph -2.50/ Cyl -5.0 axis 180. Her pachymetry was 510 ยต. She underwent uneventful PRELEX (Phaco-Refractive Lens Exchange) with toric Acrysof IQ lens implantation Rt eye.

Results:

At the 2nd post-operative day, her MR was Sph +1.0/ Cyl -2.75 axis 150 and her UCVA was 20/40. 6 days later, after I did the Lt. eye, I did rotation of the IOL for the Rt eye. Dialing of the IOL was done 20 degrees clockwise under vesico elastic material after pupil dilatation and re-localization of the axis pre and intra-operatively. At the 2nd day her MR dramatically improved to Sph 0.00/ Cyl - 0.25 axis 150 and her UCVA was 20/20 and remained stable within the same value for 3 months post-operatively.

Conclusions:

Toric IOL readjustment can be a useful management of mis-alignment of the IOL axis due to any reason. It is preferred to be done as early as possible post-operatively to avoid the adhesion within the bag and consequently facilitate the rotation. FINANCIAL DISCLOSURE?: No
