Purpose:
To evaluate visual and refractive long-term outcomes after implantation of the phakic Toric Implantable Collamer Lens (Visian TICL™, Staar Surgical, Nidau, Switzerland) for the correction of myopic astigmatism.

Innsbruck Medical University, Department of Ophthalmology, Austria
Methods:
This retrospective study included 53 eyes from 31 consecutive patients who underwent TICL implantation for the correction of myopic astigmatism between 2003 an 2011. Mean age of the patients was 38.06 years (range 24-54). Patients were evaluated early postoperatively (three to six months) and up to nine years postoperatively regarding refractive predictability, visual acuity, efficacy and stability.
Results:
Refractive errors ranged from -3.0 to -13.75 diopters (D) (mean -8.31 D) myopia with up to 6.75 D astigmatism (mean: 2.57 D), the power of the implanted TICL ranged from -9.0 to -23 D (mean: -14.61 D) spherical correction and 1.0 to 6 D (mean: 3.47 D) cylindrical correction. Expected postoperative spherical equivalent (SEQ) ranged from -0.49 to +0.15 D (mean: -0.08 D±0.17) according to the TICL power calculation formula. Mean residual spherical equivalent error was -0.2405 D (range +0.815 to -1.93 D) early postoperatively, and -0,2393 D late postoperatively. Residual cylindrical error ranged from 0.25 to 1.125 D (mean: 0.87 D ±0.39). Mean visual acuity improved after TICL implantation and over time. The efficacy index was 1.028±0.43 early postoperatively and 1.039 ±0.26 in long-term follow-up results, respectively. In two eyes reposition of the TICL was required due to rotation of the toric ICL within the first weeks postoperatively, in further two eyes exchange of the toric ICL to a spheric ICL was performed without complications.
Conclusions:
In our study, long-term follow-up results after TICL implantation showed stable refractive and visual outcomes. Rotation of the TICL postoperatively may be a challenge, but can be managed by either reposition of the TICL or exchange to a spherical ICL with the option of consecutive laser refractive surgery of the residual astigmatism.
