Purpose:
To find out various presentations of PHPV and Surgical difficulties
Setting:
Tertiary eye care centre in India
Methods:
cases of PHPV who presented to us in Lens clinic were evaluated. Their presenting features, difficulties in diagnosis and variations in surgical techniques were noted. Their post-operative course was studied.
Results:
: We had 13 cases of PHPV which formed about 1.2% of our total lens clinic patients in 2008-2010. Of these 46% were males and 20% were bilateral. The mean age for these cases was 4.61 + 3.53 years. Atypical presentation like absence of micro-ophthalmos/ microcornea were also seen. Though most cases were of combined anterior and posterior PHPV, there were cases of isolated anterior PHPV also. Surgical challenges included problems in pupillary dilatation and posterior synechia in these cases caused difficulty in anterior capsulorrhexis. Posterior capsulorrhexis was best managed with use of forceps and plasma blade. Decision for primary IOL was based on axial length and corneal diameter.
Conclusions:
: prognosis in cases of PHPV remains guarded even with current surgical advances. Use of Plasma blade is helpful for managing posterior capsule and vascular component in these cases safely. FINANCIAL DISCLOSURE?: No
