Visual performance differences between toric implantable collamer lens (ICL) and spherical ICL with corneal relaxing incision for astigmatism correction

Purpose:

To compare visual performance with toric ICL and with spherical ICL plus corneal relaxing incision for moderate myopic astigmatism correction.

Setting:

University of Valencia, Spain.

Methods:

We used an adaptive optics visual simulator (crx1, Imagine Eyes, France) to simulate the wavefront aberration pattern of a toric ICL and the wavefront pattern of a spherical ICL combined with corneal relaxing incision. Visual acuity (VA) at different contrasts 100%, 50% and 10% and contrast sensitivity function (CSF) at 10, 20 and 25 cycles/degree (cpd) were evaluated for 3 and 5-mm pupils. Modulation Transfer Function (MTF), Point Spread Function (PSF) and Strehl ratio (SR) were calculated for a 5-mm pupil.

Results:

Ten subjects were enrolled in this study. For 3mm of pupil size, toric ICL provides better VAs and CSF values at all frequencies studied than spherical ICL plus corneal relaxing incision (P< 0.05). At the same time, for 6 mm of pupil diameter, the best VAs and CSF values were obtained with the toric ICL (P< 0.05). The MTF obtained with spherical ICL plus corneal relaxing incision was worse than the toric ICL MTF. The toric ICL showed less spread out of PSF than spherical ICL and corneal relaxing incision combination.

Conclusions:

The results of the current study suggest that toric ICL provide a better visual performance than spherical ICL plus corneal relaxing incision. FINANCIAL DISCLOSURE?: No
