Panfocal visual acuity in patients undergoing mono multi-vision for emmetropic presbyopia

Purpose:

Mono Multi-Vision is a technique which can provide a permanent solution in emmetropic presbyopes who enjoy good uncorrected distance vision but have severely compromised near vision. This technique involves the implantation of a full zone One Piece Aspheric Diffractive Multifocal IOL in the non dominant eye of emmetropic presbyopia patients. The unoperated dominant eye continues to provide excellent distance vision. Uncorrected binocular near vision is improved reducing the need for near vision spectacles. This report presents the functional vision assessment in a consecutive continuos cohort of Mono Multi-Vision patients.

Methods:

10 consecutive patients who underwent mono multi-vision for emmetropic presbyopia were identified. PANFOCAL VA a validated measure of Functional vision was measured in all patients at least 4 weeks after surgery. Monocular uncorrected and corrected distance (6 metres), intermediate (66cm) and near (40 cm) snellens visual acuity in photopic conditions was also measured. Their ability to perform standard intermediate and near vision tasks like work on laptops and desktops, read books, newspapers and magazines was assessed in both mesopic and photopic conditions. A patient questionnaire was used to assess their subjective perception of their vision without spectacles.

Results:

All patients achieved binocular uncorrected distance, intermediate and near acuities of 6/9 or better in photopic conditions. Median Panfocal visual acuities were 6/9,6/9,6/9,6/9 for photopic distance, intermediate, near and mesopic near vision respectively. The key determinant of intermediate vision was the level of presbyopia in the dominant unoperated eye. The operated non dominant eye filled in the gaps for near vision very well and all patients were able to fuse the two images without any problems. Objective task performance revealed good intermediate task performance and excellent near vision task performance. Questionnaire results supported the objective and functional findings with a high level of spectacle independence and overall patient satisfaction.

Conclusions:

Mono Multi-Vision provided excellent functional vision in patients with emmetropic presbyopia.
