Purpose:
To study results of penetrating and lamellar keratoplasty (KP) in patients with hereditary corneal dystrophies.
Setting:
The S. Fyodorov Eye Microsurgery Complex - Federal Budget State Institution, Moscow, Russia
Methods:
Types of disorders included lattice dystrophy (22 cases), granular dystrophy (9 cases) and macular dystrophy (6 cases); 37 procedures of penetrating keratoplasty (8 mm disc diameter) was performed in patients 17- 35 years (30 cases of penetrating corneal transplantation and 7 cases of lamellar keratoplasty).
Results:
In all cases we registered transparent graft with high visual acuity. Nevertheless 5-10 years after surgery similar changes in the graft analogous to those at the host cornea before KP started to develop. Recurrence of the dystrophy signs in the graft at penetrating KP group appeared 1-3 years later compared to lamellar transplantation. 4 patients with the longest follow-up (30-32 years) were operated three times with 8-10 years interval.
Conclusions:
Keratoplasty in hereditary corneal dystrophies provides excellent functional and biological results. But 5-10 years after successful KP lesions typical for primary dystrophy recur.
