Purpose:
Our experience regarding Ex-press valve implantation under scleral flap with adjunct topical application of mitomycin c in patients with uncontrolled glaucoma.
Setting:
Saint loukas opthalmological department
Methods:
Ex-press valve type P-50 was implanted under scleral flap in 38 patients 22 male 16 female with mean age 64 years old. 21 patients had exfoliative glaucoma, 9 open angle glaucoma ,6 juvenile glaucoma, 1 inflammatory and 1 neovascular. The mean preoperative intraocular pressure (IOP) was 27.8 mmHg. Mitomycin c was applied in all cases. All surgeries were performed by the same surgeon.
Results:
For a mean time of 6 month follow up (ranging from 3 to 11 months) the mean postoperative IOP was 10.8 mmHg. Out of 38 cases the 32 (84%) had IOP controlled (<20 mmHg) without medical treatment while the rest 6 had IOP controlled with monotherapy. There were no serious operative and postoperative complications.
Conclusions:
Ex-press valve implantation under scleral flap with adjunct topical application of mitomycin c compared to trabeculectomy with topical application of mitomycin c has similar results regarding the IOP control but is a safer choice due to a reduction of serious complications such us fibrosis and encapsulation which are more common especially in the first 3 months after trabeculectomy.
