Purpose:
To grade the nuclear cataract using LOCS III classification system and its correlation with maximum nuclear density on scheimpflug imaging and phaco power consumed.
Setting:
Advanced Eye Center, Post Graduate Institute of Medical Education and Research, Chandigarh, India
Methods:
Nuclear density of every patient was evaluated by comparing slit lamp images with standard pictures according to LOCS III classification system. Maximum nuclear density was calculated using scheimpflug imaging system. All patients were underwent phacoemulsification and posterior chamber intraocular lens implantation. LOCS III grading and maximum nuclear density were compared postoperatively with phacoemulsification time, power consumed, balanced salt solution used and pre and postoperative visual acuity at 4 week in LogMAR units prospectively.
Results:
One hundred patients were evaluated. There was positive linear correlation between LOCS III and scheimpflug maximum nuclear density. ( P<0.001). Both LOCS III grading and scheimpflug maximum nuclear density have linear correlation with preoperative BCVA (r=0.588 and 0.525), cumulative dissipated energy (r=0.607 and 0.847), total ultrasound time (r=0.627 and 0.802), fluid used (r=0.623 and 0.678) and post operative BCVA at 4 week (r=0.383 and 0.308) respectively.
Conclusions:
LOCS III is an economical nuclear cataract grading system with satisfactory concordance with scheimpflug imaging. However, high reproducibility and level of detail about lens density suggested by the scheimpflug system make it more advantageous to integrate scheimpflug grading into routine operative planning in well equipped centers.
