Visual outcome following subluxated lens removal in Marfan syndrome

Purpose:

To report the results of subluxated lens extraction with or without iris clip lens implantation in two generations of a family with Marfan syndrome.

Setting:

University of Szeged, Department of Ophthalmology, Szeged Hungary

Methods:

Four eyes of two patiens with Marfan syndrome (mother and daughter) were operated on because of subluxated lens. Mother had axial myopia (AL: 28.0 mm) and subluxated lens with incipient cataract in both eyes. Intracapsular lens extraction (ICCE) was performed without lens implantation. Daughter had subluxated lens with normal axial lenght. Following ICCE, VERISYSE iris clip lens was implanted in both eyes.

Results:

In the first case preoperative best corrected visual acuity was 0.15 on both eyes. Following ICCE best corrected visual acuity was 0.9 on right eye and 1.0 of left eye. The follow up time was 17 and 12 years for the eyes respectively. In the second case the visual acuity improved from 0.25/0.25 to 0.95/0.65 postoperatively with ICCE and VERISYSE lens implantation. The follow up time was 1 year for both eyes.

Conclusions:

In Marfan syndrome amblyopia, lens subluxation, myopia, retinal detachment and secondary glaucoma can cause low vision. Uncomplicated surgery with or without lens implantation may result in significant improvement in visual acuity following extraction of subluxated lens in patients with long standing low vision due to Marfan syndrome.
