Clinical results of diffractive multifocal and accommodative IOL implantation in cataract patients and patients with high ametropia and presbyopia

Purpose:

To evaluate and compare the visual outcomes and accommodative amplitude after phacoemulsification with diffractive multifocal and accommodative IOL implantation in patients with non-complicated cataract or clear lens extraction.

Setting:

Eye Center «East Sight Recovery», Moscow, Russia.

Methods:

This study comprised 148 pseudophakic eyes of 96 patients with non-complicated cataract or high ametropia and presbyopia. 44 patients (62 eyes) (Group 1) received the accommodative IOL Crystalens HD-500, 52 patients (86 eyes) (Group 2) received the multifocal AcrySof ReSTOR IOL (SN6AD1). Postoperatively, refraction, uncorrected visual acuities (VA) for far, near and intermediate distance in photopic and mesopic conditions, amplitude of accommodation and pseudoaccommodation with defocus curve, level of aberrations were analyzed.

Results:

Both groups showed comparative high level of UCDVA (0,82+0,1; 0,85+0,09, p>0,05) and target refraction (+0,5 D) in 83%-85%. UCNVA 20/40 (J3) or better was achieved in 59% Crystalens patients and 75% ReSTOR patients ; intermediate visual acuity 20/30 or better - in 70% Crystalens patients and 50% ReSTOR patients. The amplitude of accommodation and pseudoaccommodation, determined with a defocusing technique and subjective near point was 3,23±0,26 D and 2,14±0,28 D, p<0,05 respectively . In mesopic conditions the lower level of decrease of UCNVA was in accommodative IOL group (p<0,05). Aberrometry analyze showed increase (on 10,6%-10,9%, p>0,05) of abberations (high order, total) in pupilar size 6 mm in accommodative IOL. Both groups showed similar (11,5% ? 13,5%) level of photic phenomena.

Conclusions:

The multifocal IOL provided better near visual acuity and the Crystalens HD-500 accommodating IOL provided better intermediate visual acuity in pseudophakic patients. The patients of both groups had similar high level of subjective satisfaction. FINANCIAL DISCLOSURE?: No
