Comparison of the Endosaver with non-injector techniques in descemet stripping endothelial keratoplasty

Purpose:

To describe and evaluate the use of an injector device the Endosaver for inserting the graft in manually dissected descemet-stripping endothelial keratoplasty (DSEK) and compare it with Non injector insertion techniques.

Setting:

Southampton University Hospital NHS Trust

Methods:

Interventional case series. Assessment of outcomes of DSEK using the Endosaver injector compared to non-injector (forceps or glide). Postoperative specular microscopy and manifest refraction at 12 months post-operatively were evaluated

Results:

No significant statistical difference was found in pre-operative and post-operative best corrected visual acuity between the Endosaver and non-Endosaver insertion groups. Mean pre-operative endothelial cell count was 2667 (±130) for the Endosaver group and 2621 (±102) for the non-Endosaver group. Post-operative endothelial counts showed a significant difference between the Endosaver 1593 (±567) and the non-Endosaver 1017 (±468) insertion groups.

Conclusions:

The Endosaver device allowed less trauma during graft insertion due to the minimal touch technique employed. A smaller insertion wound of 4.5 mm compared to non-injector cases enabled a more stable system during surgery. Mean postoperative donor endothelial cell counts were higher in the Endosaver group (P= 0.015) suggestive of less endothelial damage during insertion. FINANCIAL DISCLOSURE?: No
