Purpose:
In order to prevent endophthalmitis after cataract surgery, ophthalmic preoperative disinfection is important, which reduces the bacterial flora in the external eye including the conjunctival sac and eyelids. The disinfection efficacy of 1.5% Levofloxacin (LVFX) ophthalmic solution is expected to be enhanced in terms of tissue penetration. This study was conducted to evaluate the efficacy and the safety of 1.5% LVFX ophthalmic solution in ophthalmic preoperative disinfection with a multi-center prospective study.

Methods:
We enrolled patients who were scheduled for cataract surgery. The perioperative regimen of 1.5% LVFX ophthalmic solution was as follows: Preoperational period: 3 times per day for 3 days; The operating day: once in the morning, once 1-hour before the operation, and just after the operation; Post-operational period: 3 times per day for 14 days. Conjunctival sac scrapings for bacteriological examination were collected 3 times: before LVFX application, before the eyewash with iodine compounds on the operating day, and after the 14-day LVFX application.
Results:
So far 39 patients were enrolled this study. The mean age was 73.7 years. The eradication rate of patients showing a positive culture prior to LVFX application was 85.3% (29/34 cases). The preoperative culture-negative rate was 87.2% (34/39 cases). The postoperative culture-negative rate was 87.2% (34/39 cases). The frequently isolated aerobes were S.epidermidis and Corynebacterium species, which had preoperative eradication rates of 100% (14/14 cases) and 88.2% (15/17 cases), respectively. The frequently isolated anaerobe was P.acnes, which had a preoperative eradication rate of 83.3% (10/12 cases). Adverse drug reactions were eyelid swelling (1/40 case) and very slight superficial punctate keratitis (1/39 case).
Conclusions:
The above interim result suggests 1.5% LVFX ophthalmic solution has a sufficient antibacterial effect and a good safety profile for use in ophthalmic preoperative disinfection.
