Purpose:
We have performed secondary IOL implantation by a scleral suture fixation for many cases in these 20 years, and examined recent cases about postoperative complication and visual function.
Setting:
Forty-five eyes of 44 cases underwent a scleral suture fixation of IOL at Fukuoka University Chikushi Hospital and Kitakyushu City Yahata Hospital from 2005 to 2010. The ages at a scleral suture fixation of IOL were between 38 and 88 year-old. The reasons of a scleral suture fixation of IOL were as follows; Lens luxation spontaneously in 4 eyes. Posterior capsule rupture and/or dropping of lens nucleus during the PEA surgery in 9 eyes. IOL dislocation after the cataract surgery with IOL implantation in 10 eyes. Aphakia after ECCE or ICCE without IOL implantation in 4 eyes. Post-vitrectomy eyes without IOL implantation in 10 eyes. IOL luxation after vitrectomy with IOL implantation in 7 eyes.
Methods:
Almost all of the eyes were done the scleral suture fixation of IOL using 10-0 nylon setting up an infusion plug. To perform a vitrectomy, 20G-three port system was set. In cases of IOL or lens nucleus dropping on the retina, Perfluordecalin was used to lift them from the retinal surface. In cases of aphakic eyes with vitreous, anterior vitrectomy was done before the sclera suture fixation of IOL.
Results:
The follow-up period after the surgery was from one to 24 months. Main postoperative complications were hyphaema, intravitreal bleeding, transient high intraocular pressure. Two eyes underwent cleaning of anterior chamber because of hyphaema with high intraocular pressure. They were post-vitrectomy eyes for proliferative diabetic retinopathy. Some eyes had IOL tilting slightly after the surgery. There were 2 eyes with visual acuities down more than 2 lines after the surgery. The follow-up periods of these 2 cases were within one month. All cases except 2 had stable or improved visual acuities compared with preoperative visual acuities.
Conclusions:
All cases were performed IOL scleral suture fixation without any trouble and there were no severe complication after the surgery.
Setting of an infusion plug was useful to keep the intraocular pressure and to perform the surgery safely. FINANCIAL DISCLOSURE?: No
