LEC removal by IOL rotation: effect on PCO after four years

Purpose:

To evaluate the effect of mechanical LEC removal on PCO occurrence compared to conventional capsule polish. Square edge IOL`s perform scraping mechanical effect on both capsules during their tilt and rotation in the bag. Removal of LEC is obvious as they form strands of 'cortex like' material which can than be easily aspirated.

Setting:

Professional Eye Hospital, Zemun, Serbia.

Methods:

36 patients with bilateral cataract surgery were included. Surgeries were 1-3 weeks apart, performed through 2.65 clear corneal incision. Capsulorhexis was 5.0-5.5mm and phacoemulsification was performed using 'quick chop' technique followed by bimanual I/A. On one eye we performed conventional capsule polish before IOL implantation. On the other eye we rotated IOL in the capsular bag for 3 full circles during viscoelastic removal. IOL's implanted were identical in both eyes: 360 degree square edge aspheric (40 SofPort and 32 Akreos AO, Bausch&Lomb). Follow up was 48 months and data collected was on lines lost due to PCO and YAG rate.

Results:

Average age of patients was 62.2 years. Average surgery time did not differ significantly between groups. There were no complications. Nine patients lost one line, five lost two lines and there were seven YAG capsulotomies on conventionally operated eye after 48 months due to PCO. In the same period, in 'IOL rotation' group, only three eyes lost one line.

Conclusions:

The maneuver of multiple IOL rotations in the bag is very effective way to eliminate LEC even in distal capsular areas. It is performed during and after viscoelastic removal and it does not prolong surgery time. FINANCIAL DISCLOSURE?: No
