Surgical treatment of ectopia lentis and congenital cataract in children with Marfan's syndrome: evaluation of IOL fixation types

Abstract Details

Purpose:

To analyze the results of surgical treatment and IOL fixation types in children with congenital cataract and Marfan`s syndrome

Setting:

Excimer Eye Centre, Moscow, Russian Federation

Methods:

The study included 14 patients (27 eyes) aged 3-16 years (7.8±4,1) with Marfan`s syndrome and congenital cataract, treated in Excimer eye clinic in 2003-2015. BCVA after surgery was 0.1-0.4 (0.32±0.16). All patients conducted cataract phacoemulsification with IOL implantation after a standard ophthalmological examination. AcrySof SN60AT (n=2), SN60WF (n=3), MA60AC (n=17), MA60ВМ (n=2) and toric SN60T5 (n=3) were implanted. Type of IOL fixation depended on lens ectopia degree, capsular characteristics and child` age.

Results:

3 main IOL fixation types were determined: mixed (n=5), intracapsular (n=14) and intrascleral (n=8). BCVA in 12-36 months after surgery was 0.4-1.0 (0.78±0.26). We observed IOL dislocation in 5 cases. In the mixed IOL fixation group displacement of IOL was in 3 cases without suturing and membranous cataract in 2 cases. In the intracapsular group there were 6 cases of IOL dislocation and 7 secondary cataracts. After intrascleral IOL fixation one eye needed lens reposition. We did not observe uveitis, retinal detachment, macular edema etc. in our patients.

Conclusions:

Children with Marfan`s syndrome noted a significant improvement in visual function after lensectomy with IOL implantation. Performing surgery in preschool children, in spite of the complexity of the surgical technique, leads to clinically significant functional results and successful treatment of amblyopia. The least frequency of postoperative complications was determined in the group with the intrascleral IOL fixation, thus the difference was not significant due do the low number of patients.
