Anterior corneal surface aberrations and asphericity following LASIK by optimised ablation profile for high hyperopia up to +8.5D

Abstract Details
Purpose:
To evaluate corneal higher order aberrations (HOA) after LASIK (500 Hz repetition rate) and optimised ablation profile. To asses the optical quality of the cornea after lasik for high hyperopia up to +8.5D to determine if it can be a proper alternative treatment to refractive lens exchange or phakic IOLs.
Setting:
Vissum instituto oftalmologico, Alicante, Spain
Methods:
Retrospective study including 51 eyes with high level of hyperopia (mean SE +5.64D) with a 6-months follow-up. All cases underwent LASIK using the femtosecond platform for flap creation.
Results:
Significant increase in Root Mean Square (RMS) HOA, and RMS-COMA, while a decrease in spherical aberration were observed (p<0.01). Asphericity for the 4,5mm (Q45) and 8mm (Q8) of corneal diameter changed significantly (p<0.01), with no change in Strehl ratio (p =0.77)
Conclusions:
LASIK significantly induces corneal HOAs. Increase in RMS-COMA is the most important factor related to a poor optical outcome. Induction of coma is the most important limiting factor in LASIK for high hyperopia between 5 and 8.5D. Improvment in terms of safety is needed in order to consider LASIK in such high hyperopic cases.
