Comparative analysis of efficacy of combining photorefractive keratectomy (PRK) with cross-linking versus standard PRK

Abstract Details

Purpose:

The purpose of the study was to look into the efficacy, refractive accuracy and safety of combining PRK with Cross linking(CXL) as compared to standard PRK without any change in the treatment algorithms for PRK.

Setting:

Cornea and Refractive services at a tertiary eye care hospital in south India

Methods:

Retrospective comparative interventional study. For the PRK with CXL group(Group A), all patients with borderline suspicious tomography on a Scheimpflug based tomographer or low pachymetry (<470 microns) were included. A comparative group who were matched for Age and spherical equivalent but with normal tomography and pachymetry were taken in the standard PRK group(Group B). At the end of the PRK procedure, Group A underwent accelerated cross linking 30mW/cm2 for 90 seconds. In Group B, 0.02% Mitomycin C was applied for 30 seconds at the end of the procedure. Minimum follow up period was 6 months.

Results:

30 eyes 15 patients were included in Group A and 50 eyes of 25 patients were included in Group B. The mean age was 24.5 & in Group A & 23.56 in Group B. The mean spherical equivalent was -3.42 in Group A and -3.3 in Group B. Group A had significantly higher Mean K and K max (p<0.001). 100 percent of patients in Group A maintained BCVA as compared to 98 percent in Group B. The mean residual spherical equivalent was 0.15 D in Group A and 0.65 D in group B. No haze was noticed in any patient in Group A , despite lack of use of Mitomycin C, and I patient developed haze

Conclusions:

Crosslinking in suspicious tomographies in Laser vision correction without a commensurate refractive change augurs for biomechanical integrity and predictable refractive accuracy with no resultant drop in quality of vision.
