Early result of topography-guided photorefractive keratectomy for keratoconus

Purpose:
To evaluate early results of topography-guided Photorefractive Keratectomy (TGPRK) for keratoconus with Schwind Amaris.
Setting:
Pacific Laser Eye Centre, Vancouver, BC, Canada
Methods:
Retrospective series. Evaluate outcome of keratoconus eyes underwent treatment with Schwind Amaris Excimer laser. Pre-operatively and post-operative uncorrected visual acuity (UCVA), corrected distance visual acuity (CDVA), manifest refraction (MR) and topographic cylinder were analyzed. Eyes with at least 12 months of follow-up were included.
Results:
254 eyes with keratoconus underwent treatment with 133 had sufficient data at 12 months for analysis.
71 of 133 (53%) showed UCVA ≥20/40 post-operatively. 61 (46%) had improved CDVA and
29 eyes (22%) gained two or more lines (p=0.0003). No patient showed progression.
Mean astigmatism was reduced from 3.06±1.83D to 1.64±1.58D (p<0.0001).
Mean spherical equivalent was improved from -3.07±3.85D to -0.88±2.68D (p<0.0001).
Conclusions:
Early results of TG-PRK with Schwind Amaris show efficacy and safety as treatment for eyes with keratoconus. More than half had UCVA 20/40 or better post-operatively. Almost half had improved CDVA. Almost one quarter had CDVA improved two or more lines. This study demonstrates an alternative treatment for keratoconus with contact lens intolerance.
