Long-term functional and morphological outcomes and patient satisfaction after cataract surgery with progressive apodised diffractive multifocal intraocular lens (PAD-Mf-IOL) implantation with/without posterior central circular capsulorhexis (PCCC)

Purpose:
To evaluate long term visual and morphological outcomes as well as patient satisfaction and spectacle independence after PAD Mf-IOL implantation in patients with cataract, operated with and without PCCC in a progressive cohort study.
Setting:
Csolnoky Ferenc University Teaching Hospital, Veszprem, Hungary
Methods:
Fifty cataract patients with presumably healthy fundi had bilateral phacoemulsification and PAD-Mf-IOL (BiFlex-M) implantation. For each patient one eye was operated on with PCCC whereas the fellow eye without PCCC. Six and 12 months postoperatively, the position of the lens was tested and photographed with undilated and dilated pupil. The uncorrected (UC-) and corrected (C-) distance (DVA), intermediate (IVA), and near (NVA) visual acuities; residual refractive errors; mesopic defocus curve; mesopic, photopic and backlight contrast sensitivity were tested. Patient satisfaction was evaluated with VFQ-25 test, with particular regard to visual artefacts.
Results:
Follow up of 6 and 12 months were completed by 97 eyes so far. Mean 6/12 months logMAR visual acuities were UC-DVA: 0.026/0.03, UC-NVA: 0.099/0.099, and UC-IVA: 0.06/0.058. Residual refractive error of less than ±0,50 diopter was achieved over 90% for all distances. Contrast sensitivity levels were high. Backlight glare-halo-starburst symptoms were reported mild/moderate/serious in 40/8%/2% respectively. Spectacle independency for distance and near was achieved in 100%, and 97% for intermediaer vision. IOL-s rotated 3.61 ±2.34 degrees between postoperation day and 1 months, 0 degree thereafter. Patient satisfaction rate were 94-95% respectively. PCCC did not influence functions or lens stability.
Conclusions:
Long term results of PAD-Mf-IOL (BiFlex-M) implantation provided nearly total glasses independence for distance, intermediate and near vision. Visual artefacts were of low level and easy to tolerate, improving with time. The position of the lens in the bag was stable. In the observed timeframe PCCC did not seem to have influence to visual functions and lens position and there was no need for YAG-capsulotomy. Suboptimal functions were measured only in patients with minor non lens/IOL-related (mostly macular) alterations discovered during follow up.
