Refractive lensectomy with bilateral implant of a new diffractive trifocal lens

Abstract Details

Purpose:

To assess the efficacy, predictability and safety of the non-apodized, aspheric trifocal intraocular lens (IOL) PanOptix (Alcon, Fort Worth, Texas) for near, intermediate and distance vision in bilateral surgery of the crystalline lens.

Setting:

Instituto Universitario Fernández-Vega (Oviedo). Spain.

Methods:

Clinical trial that enrolled 32 patients (64 eyes) who were bilaterally implanted with PanOptix IOL. The inclusion criteria were: preoperative sphere in the range of correction of the lens, refractive cylinder lower than 1 dioptres (D) and 3 months of minimum follow-up. Capsulotomy was performed with Lensex (Alcon, Texas – U.S.A) femtosecond laser. After phacoemulsification the IOL was implanted in the bag. Refraction, distance corrected visual acuity (DCVA) and bilateral defocus curves with patient’s distance corrected vision were analysed. Astigmatism was assessed by the Thibos’ method and the refractive indexes (efficacy, safety and predictability) were calculated.

Results:

: At three months from surgery the manifest sphere had diminished from +0.84±1.76 to -0.04±0.53 D and the refractive cylinder had little variation from -0.36±0.39 to -0.33±0.50 D. Uncorrected and corrected distance visual acuities improved from 0.37±0.26 to 0.74±0.26 and from 0.72±0.23 to 0.91±0.11 (decimal scale), respectively. Efficacy index was 1.03. All eyes kept or improved their initial DCVA, with 63.2% of the eyes improving. Safety index was 1.26. 85% of the eyes were between ±0.50 D of sphero-equivalent and 100% between ±1.0 D. Defocus curve analysis showed excellent distance vision and homogeneous range of vision between 33 and 100 centimetres.

Conclusions:

The results in terms of efficacy, safety and predictability of the PanOptix IOL were good at distance, intermediate and near vision during the follow-up. Indexes over 1.0 suggest that the lens is a good choice for lensectomies programmed with a refractive purpose.
