Light distortion and quality of vision in binocular implantation of three different IOLs for presbyopia correction

Abstract Details

Purpose:

Nowadays a wide variety of lenses to address presbyopia are available in the market. These include multifocal lenses that provide two or three focal points using diffraction principles and also lenses that use polyfocality to smoothly extend depth of focus (EDOF). The purpose of this study was to evaluate the visual performance of three novel multifocal IOL, under challenging conditions involving low lighting and glare sources.

Setting:

The present study was conducted at the Ophthalmology Center at the Hospital da Luz, in Lisbon, Portugal.

Methods:

The 43 patients were evaluated after binocular implantation (1 week between both surgeries) of multifocal IOLs: 23 of them with an average age of 62.48±7.97 years of age received FineVision PODF (PhysIOL), 15 (age 63.36±9.19 years) were implanted with Symfony (AMO-Abbot) and 5 (62.96±9.17 years) were implanted with PanOptix (Alcon). A comprehensive ophthalmic evaluation was conducted (45, 90 ± 31 days after last surgery) followed by evaluation of defocus VA (from -3.00 to +1.00 in 0.50 diopters steps), Photic Phenomena (LDA), Contrast Sensitivity (photopic and under glare conditions) and subjective Quality of Vision questionnaire (QoV).

Results:

3 groups of patients achieve optimum performance in terms of VA at distance. The EDOF lens had better results in intermediate vergences when compared with the other two trifocal IOLs. No relevant differences were found in contrast sensitivity, both in photopic and night glare conditions. Frequency, severity and bothersome of night vision complaints were very similar among the 3 groups, but those with EDOF lenses related more bothersome (average score: 47.20) than the trifocal groups. LDA showed high levels of average disturbance index for Symfony (34.64%) compared with FineVision (27.44) and PanOptix (28.65).

Conclusions:

Subjective and psychophysical evaluation of the quality of vision is useful in detecting differences between the performance of different IOL designs for presbyopia. Considering the differences found in terms of night vision disturbances, it is very important to select the appropriate IOL according to patient’s lifestyle and expectations in order to avoid complaints such as those related with night light conditions which are one of the major causes of dissatisfaction after IOL implantation.
