Objective and subjective visual performance of multifocal implants: trifocal technology versus elongated focus

Abstract Details

Purpose:

To compare clinical outcomes and visual quality parameters obtained after implantation of two multifocal intraocular lenses (IOL): the trifocal lens AT LISA® tri 839MP and the elongated focus lens TECNIS Symfony ZXR00.

Setting:

Hospital Clínico Universitario Lozano Blesa, Zaragoza, Spain.

Methods:

This prospective series of cases study comprised forty-eight cataractous eyes from 24 patients who underwent crystalline lens phacoemulsification with multifocal IOL implantation. Fourteen patients were bilaterally implanted with elongated focus IOL (group 1) and were compared with ten patients bilaterally implanted with trifocal IOL (group 2). Near, intermediate and distance visual acuity (VA) using the ETDRS chart and contrast sensitivity were measured after surgery. Optical quality parameters obtained were modulation transfer function cut-off frequency (MTF cut-off), objective scattering index (OSI), root mean square of total aberrations (RMS total), RMS of higher order aberrations (RMS HOAs) and spherical and coma aberrations.

Results:

We found statistically significant differences between groups for the near VA without correction (p<0.0, U-Mann-Whitney) with better results for group 2. And in most of the spatial frequencies of contrast sensitivity in mesopic conditions (p= 0.038; p= 0.014; p< 0,0 and p< 0.0 for 3, 6, 12 and 18 cycles per degree respectively, U Mann-Whitney) , obtaining the best results in group 1. The postoperative average values of MTF cut-off, RMS, RMS HOAs and spherical aberration and coma showed no statistically significant differences between groups. The OSI index exhibit values significantly higher in group 2 (p =0.007, U Mann-Whitney).

Conclusions:

Both implants allow the optimal restoration of vision after cataract surgery, providing excellent distance and intermediate vision. The optical design of each implant determines different behaviour in both, objective and subjective parameters. The near focus of trifocal IOL allow better results for near vision. However, trifocality could be responsible for the increase in intraocular scattering in these patients, with worse outcomes in contrast sensitivity test, especially in mesopic conditions.
