Early experience with a new single piece diffractive trifocal intraocular lens

Purpose:
To evaluate the efficacy of a new single piece diffractive trifocal intraocular lens (IOL) in reducing spectacle dependence in patients undergoing cataract surgery.
Setting:
Multi-disciplinary private hospital.
Methods:
Prospective case series study. Inclusion criteria were eyes with no other co-morbidities. A total of 27 eyes in 18 patients were recruited. Of these, 9 patients had bilateral implantations of the PanOptix trifocal IOL (Alcon Laboratories) and 6 patients had unilateral implantations combined with a ReSTOR bifocal IOL in the contralateral eye. A single surgeon performed all operations, with no intra-operative complications. Preliminary results at one month post-op are included. Outcome measures were (i) distance, intermediate and near visual acuities (ii) defocus curves (iii) patients’ perceived symptoms of haloes and glare (iv) patients’ spectacle independence and overall satisfaction.
Results:
85% of eyes could see 20/30 or better unaided. When tested binocularly, all patients could see 20/25 or better for distance, intermediate and near. All bilateral PanOptix patients could read J1 for near/intermediate, and J3 or better under dim light conditions. Defocus curve evaluations showed a continuous sustained curve of better than 20/25 vision across 35 to 60 cm distances. Haloes were reported by 89% of patients but none were affected significantly and all were able to drive at night with no difficulties. All patients were spectacle independent for all distances and were able to perform their daily activities with very little difficulties. A high level of satisfaction was noted in all patients.
Conclusions:
This new trifocal IOL was found to be effective in providing spectacle freedom for near, intermediate and distance vision. Stand-out features were (i) its provision of good vision from near to intermediate distances in a smooth continuous manner and (ii) its near vision capability under dim light conditions. The lens seems to be well tolerated. Patients’ overall satisfaction was high.
