Quality of life perception and reading quality after implantation of multifocal and monofocal intraocular lenses

Purpose:
To compare NEI RQL-42 quality of life test and Radner Vissum reading test outcomes between patients with implantation of Tecnis ZMB00 multifocal intraocular lens (IOL) and patients with implantation of Tecnis ZCB00 monofocal IOL after bilateral cataract surgery.
Setting:
Miguel Servet University Hospital, Zaragoza, Spain.
Methods:
Fourty two eyes of twenty one patientswho had phacoemulsification were implanted Tecnis ZCB00 monofocal IOL and eighty two eyes of fourty one patients were implanted Tecnis ZMB00 multifocal IOL. They were asked to answer the NEI-RQL-42 quality of life questionnaire before and three months after cataract surgery. The Radner Vissum reading test was performed three months after the surgery under photopic lighting conditions. Patients with a monofocal IOL were examined with optical correction for near vision but patients with a multifocal IOL were examined without it.
Results:
An improvement was found in most of the items of the NEI-RQL-42 testbetween the preoperative and postoperative period in both groups. However, the multifocal IOL group obtained significantly better results in items number two, seven, eight, eleven, thirteen, thirty one and forty, and in the following categories: near vision, dependence on correction and suboptimal correction (P<.05). The monofocal IOL group had only significantly better results in item number seventeen.
As for the Radner Vissum test, the multifocal IOL group obtained significantly better results in phrases number one, three, four and five, and in the number of incorrect syllables (P<.05).
Conclusions:
Patients with Tecnis ZMB00 multifocal IOL relate a higher quality of life regarding the lack of needing optical correction for near vision in their daily activities.Nevertheless, the halos vision at night in the presence of lighting signs is regarded as a negative aspect by patients with multifocal IOL.Additionally, patients with multifocal IOL achieve a similar or even better reading quality at near vision and under photopic lighting conditions than patients with monofocal IOL with near vision optical correction.
