Topographic and refractive changes after pterygium surgery

Abstract Details

Purpose:

To present one case of pterygium induced astigmatism in both eyes comparing refraction and topography pre and post-surgery.

Setting:

Instituto Oftalmologico Hoyos. Barcelona (Spain)

Methods:

A 79-year- old male patient with pterygium in both eyes. Uncorrected visual acuity (UCVA) was 20/400 in the right eye (RE) and 20/100 in the left eye (LE). Best corrected visual acuity (BCVA) was 20/30 in RE with +5 -3.5 x 160º, and 20/20 in LE with +1.5 D. The topography showed an irregular astigmatism and the keratometric readings were 44.03 x 39.56 in RE and 44.63 x 44.18 in LE. We performed pterygium surgery by excision, mitomycin C and conjunctival flap reconstruction.

Results:

One month later UCVA was 20/40 and BCVA was 20/20 in both eyes. Refraction was +2.5 -2 x 110º in RE and +1.75 -0.5 x 60º in LE. The topography showed an improvement in the corneal surface regularity with a reduction of astigmatism.

Conclusions:

Pterygium could induce refractive and topographic astigmatism with a decrease of vision. Surgery must be done as soon as possible to ensure a good recovery.
