Purpose:

Spontaneous posterior capsule rupture is a rare entity. We describe such a case in a mentally retarded young man.

Results:

We present a case of a 25-year old man with congenital mental retardation of unknown origin. Ophthalmic history included microphthalmia ODS and a phacoemulsification OD with IOL implantation in 2003 for mature cataract. At that time a mild cataract OS was also diagnosed. Patient presented recently at our clinic for a regular follow up visit. Visual acuity in OD was 0.4 and in OS 0.63=. Incidentally a spontaneous ruptured posterior capsule with a luxated lens was found in the left eye. There was no ocular inflammation.

Conclusions:

We describe the case of a 25 year-old mentally retarded man with spontaneous posterior capsule rupture and lensluxation. We presume he reaches this good visual acuity by looking through the periphery of the lens using a head tilt. In the literature spontaneous posterior capsule is described, however this is mainly in elderly patients or with posterior polar cataract. No other cases are described of spontaneous posterior capsule rupture in mental retardation nor in microphthalmia. It is unknown whether eye-rubbing may have played a role in the development of the capsular rupture.
