Chronic carotid cavernous fistula: a case report

Purpose:

To describe the clinical case of a patient with a chronic low debit carotic carvernous fistula

Setting:

Outpatient clinic, Instituto de Oftalmologia Dr Gama Pinto, Lisbon, Portugal

Methods:

Case report of a single patient

Results:

We report the case of a 92 year old female presenting with decreased visual acuity on her left eye (LE). On examination, best corrected visual acuity LE was 3/10; biomicroscopy revealed conjunctival vascular turtuosity on a “medusa head” pattern. On gonioscopy grade IV Schaffer open angle was observed, with blood visible on Schlemm’s cannal. On fundoscopy, papillar and macular edema as well as exuberant retinal arterial and venous engorgement were observed. Suspecting a low debit arterial-venous fistula, the patient underwent orbital and cranial MRI scan which showed extraocular muscle enlargement, superior ophthalmic vein and left side cavernous sinus dilation, confirming the diagnosis. She was referred to a neuroradiology consult but her advanced age was deemed contraindicate to invasive intervention.

Conclusions:

CCV is a rare clinical entity which will often present with ocular manifestations which can ultimately lead to full vision loss. A high index of suspicion for patients presenting with its characteristic signs and symptoms is of indisputable importance in both establishing an early diagnosis and designing a suitable treatment plan.
