Surgical modifications in evisceration and enucleation with porous intraorbital implants

Abstract Details
Purpose:
Porous intraorbital implants sometimes cause tissue dehiscence leading to exposure and occasional explantations. To evaluate the effects of few simple and innovative surgical modifications in evisceration and enucleation with porous implants in preventing exposure.
Setting:
Orbit & Oculoplasty unit at Disha Eye Hospitals, barrackpore, Kolkata, India, since 2001.
Methods:
324 cases with synthetic porous ceramic implants since 2001 were reviewed. In first 201 cases, routine evisceration (147) and enucleation (54) were performed. Next 123 cases (69 evisceration, 54 enucleation) were subjected to innovative surgical modifications in both categories. Postoperative complications of exposure and explantation were noted. Minimum follow up period was 1 year.
Results:
14 cases altogether showed implant exposure. Explantation had to be performed in 9 cases. Enucleation procedures showed a 2.5 times higher exposure incidence over evisceration. Surgical modifications in both evisceration and enucleation significantly reduced implant exposure (p<0.01).
Conclusions:
Few simple innovative surgical modifications can be helpful to prevent the very common problem of implant exposure with porous orbital implants in both evisceration and enucleation.
