No suture no glue graft fixation in conjunctival autografting for pterygium: comparison with other conventional methods

Abstract Details

Purpose:

Conjunctival autografting is currently the most accepted procedure for pterygium surgery with its low recurrence rate and no fatal postoperative complications. Graft fixation with fibrin glue has proved superior over suturing. This study evaluates the efficacy of a simple and innovative graft fixation technique using patient’s own blood, and compares the results with the conventional ones.

Setting:

Prospective, comparative, interventional case series done at Disha Eye Hospitals, Barrackpore, Kolkata, India, since 2011.

Methods:

859 primary single pterygium operated with free or limbal conjunctival autografts from January 2011 to March 2015. Graft fixation done with sutures in 154 cases (Gr A), fibrin glue in 427 cases (Gr B) and autoblod in 313 cases (Gr C). Recurrence rate, mean surgical time and postoperative subjective symptoms in each group were recorded. Minimum follow up was 10 months.

Results:

Recurrence was maximum in Gr A with sutures (13.63%). Fibrin glue has the fastest mean surgical time (7.2±1.5 min) and postoperative recovery. The novel technique of graft fixation with autoblood showed a faster mean surgical time (11.7±0.7min) than suturing (16.3±2.4 min). The recurrence rate (2.8%) and postoperative subjective improvement rates with no suture no glue graft fixation were at par with fibrin glue.

Conclusions:

Autoblood can overcome the drawbacks of the synthetic glue, at the same time providing its benefits over suturing. So it can be a good alternative to the glue excepting a few special situations.
