Accuracy of femtosecond laser in flap and cap creation during corneal refractive surgery for myopia

Purpose:
To evaluate and compare the thickness of mechanical microkeratome, femtosecond Lasik flap and cap of small incision lenticule extraction (SMILE)
Setting:
Benha Ophthalmology Department. Benha Faculty of Medicine, Benha University
Methods:
A prospective case study was performed on 103 eyes of 52 myopic patients (20 males and 32 females). Lasik flap was done using Moria M2 microkeratome (Moria SA, Antony, France) in 30 eyes (MF Group) and using Visumax femtosecond laser (Carl Zeiss Meditec AG, Jena, Germany) in 38 eyes (FF Group) and SMILE was done by Visumax in 35 eyes (SC Group). Anterior segment optical coherence tomography (Topcon 3D OCT-2000, Jssel,NL) (AS-OCT) was done 2 weeks postoperatively to measure the flap and cap central thickness.
Results:
Mean flap thickness was 123.37-±4.41 μm in MF group, 105.03 ±2.49 μm in FF group and 105.89 ±2.21 μm in SC group (P<0.001). There were statistically significant difference between MF group and FF group (mean difference=18.34 μm, P<0.001) and also between MF group and SC group(mean difference=17.48 μm, P<0.001). There was no statistically significant difference between FF group and SC group (mean difference=0.86 μm, P=0.1241).
Conclusions:
AS-OCT showed that flaps and caps done by femtosecond laser are more accurate than flaps done by Mechanical microkeratome. The accuracy doesn’t differ between flaps of femtolasik and caps of SMILE.
